Grant Description

Contracts for services to Minnesota organizations to provide technical assistance and services to benefit individuals with disabilities. The purpose of the program is to help individuals with disabilities increase their independence within and outside of their homes. Successful applicants will provide training, outreach, technical assistance, and evaluation.

The Minnesota Department of Human Services, through its Disability Services Division (STATE), is seeking proposals from qualified Responders to use an advanced practice team approach to provide person-centered assistive technology (AT), evaluation, case consultation and technical assistance. Case consultations will be available to individuals with disabilities of all ages, their case managers, and others chosen by the participant to be part of the technical assistance team.

Consultations will include individual AT evaluations and technical assistance, and information, as well as training and outreach within the practitioner’s scope of professional practice. Training and outreach will include presentations organized by the Department of Human Services to lead agencies as well as advice to Department of Human Services, Disability Services Division workgroups upon request. These activities will help Minnesotans with disabilities that live at home, or wish to move to homes of their own and are enrolled on home care, and/or home and community based waiver services, increase their independence through the use of AT at home, at work, in their neighborhoods and across their lives.

Project deliverables must include measurable benchmarks with timelines and schedule of delivery and include at minimum:

Projections of the number of counties and tribes that will be served statewide;
Number of case consultations to be conducted per annum;
Number of presentations at conferences, including the DHS sponsored conferences per annum;
Projected consultations to advise the DHS Disability Services Division on Assistive Technology policy;
Regular required reports as designed by the Department of Human Services on project goals and deliverables; and
Marketing and outreach goals to inform counties and tribes of the services and availability of the grant

Additional Eligibility Criteria:

Grants will be available each year for qualified organization(s) to provide the above described consulting and evaluation services statewide to teams of individuals, counties, tribes, and others chosen by the participant.

Pre-Application Information:

Proposals must be submitted on January 24, 2023 by 4:00 p.m. Central Time.

DSD anticipates notifying successful applicants by February 28, 2023.

Estimated Size of Grant:

Proposals of any amount equal to or less than $1,244,616 over two years with no more than $622,308 in each fiscal year will be considered.

Term of Contract:

The term of any resulting contract is anticipated to be for two years, from July 1, 2023 until June 30, 2025 STATE may extend the contract up to a total of five (5) years.
